Certificate of Indian Blood (CIB): New applicants need to provide an official Certificate of Indian Blood to their agency office by mail or hand delivery. However, continuing applicants do not need to resubmit the CIB unless there has been a legal name change. For requesting the Office of Vital Records to forward the official CIB to the ONNSFA, applicants can use a specific form.
Official Transcripts: All applicants are required to submit sealed and unopened official high school transcripts, GED scores (if applicable), and transcripts from any colleges attended. The ONNSFA accepts official electronic transcripts (e-transcripts) that are digitally certified and delivered by authorized agents like Parchment or the National Student Clearinghouse.
